Transforming Community Services in London – TransformLDN

The TransformLDN project was established in April 2014 by NHS England London’s Chief Nurse, Caroline Alexander, in collaboration with health and care leaders and commissioners from across London. The aim was to create a vision and declaration that inspires all community practitioners and commissioners to realise a community-led transformation in health and social care in order to respond to the growing and changing needs of the capital’s patients.

Since the project launched, more than 1,000 people have shared their feedback on what excellent community services look like. These people include patients and carers; academics and opinion formers; commissioners and front-line practitioners from across London’s NHS.

This feedback has been collated and reviewed by a collaborative project design group to develop a ‘Declaration of the Foundations of Community Services in London’ and a website to support the journey of transformation.
