In 2017 Leeds Community Healthcare NHS Trust led a multi-organisation team cross West Yorkshire to success in becoming a Wave 2 New Care Model pilot site for children and young people’s inpatient mental health in West Yorkshire. Our collaboration with Bradford District Care NHS Foundation Trust and South West Yorkshire Partnership NHS Foundation Trust, now holds the budget to fund beds for young people who need a mental health admission. We work in very close collaboration and are well-supported by Leeds and York Partnership NHS Foundation Trust and the West Yorkshire and Harrogate Health and Care Partnership.

The overall aim of the pilot is to ensure that young people who need a hospital admission for their mental health are found a bed in a timely way, as near home as possible, and are discharged when they are ready to a safe package of care in the community.  We anticipate a reduction in bed usage due to the new care model, as many young people stay in hospital longer than they should do for a variety of reasons, and some are admitted when they could receive care more tailored to their individual needs in their own community. To achieve this, we have three Care Navigators working fulltime across West Yorkshire, helping to change culture and facilitate smooth hospital stays and discharges. As Clinical Lead I supervise them and lead on clinical engagement across the West Yorkshire patch. I am looking forward to introducing the Care Navigators and myself to you during the week and showing you some of our working week.
