Incident report for 2015-16

This is a part year report covering the period from the contract start on 1 September 2015 to 31 March 2016.

We know that stakeholders share our recognition of the importance of safeguarding patient and personal identifiable information in the provision of this service.  We have committed to build on the information we already publish in our annual report regarding incidents by providing further information each year on the incidents that have been reported to NHS England by Capita, as the provider of primary care support services.  All these incidents are investigated and jointly reviewed to ensure that learning is applied to improve the service delivery over the life of the contract.
