The changes being made

There are a number of programmes already underway, such as the Dorset Care Record and community hubs.

Dorset Care Record

The Dorset Care Record is an electronic individual clinical record that will, over time, include a range of shared health and social care information about a person. This shared electronic record will mean safer, more coordinated care and will mean there is no need for people to repeat their details to different professionals. It links health and social care information from GP practices, Poole and Bournemouth borough councils, Dorset County Council, Dorset County Hospital, Dorset Healthcare University NHS Foundation Trust, NHS Dorset CCG, Poole Hospital, and The Royal Bournemouth and Christchurch Hospitals NHS Foundation Trust. Community hubs

Teams and specialists work together on the same site as community hubs, sharing their knowledge to help solve problems or streamline the care that people receive.

For example, Christchurch Health Hub is made up of Christchurch Hospital, (which includes X-ray, blood and therapy services), a doctors’ surgery, a pharmacy, a care home and assisted living accommodation. Together, these services provide the community with all their non-acute care needs, saving them a journey to hospital.
