The National End of Life Care Programme, the Dying Matters coalition and the University of Nottingham, originally published this in 2009. It was revised in 2012 and republished by NHS Improving Quality in 2014 following the National End of Life Care Programme becoming part of NHS Improving Quality in 2013.

Advance care planning is a process of making people aware of your wishes when planning for end of life care. This booklet provides an explanation about advance care planning and the different options open to you. The booklet uses some of the terms contained within the framework of the Mental
Capacity Act (2005).
