This resource lists London boroughs community learning disability services (CLDS) who provide specialist health interventions for adults with a learning disability.

The resource contains links to the CLDS websites where they exist together with contact details for referrals and enquiries.

Some of the services provide either specialist physical health services or mental health services only. Some may provide both physical and mental health services. Some of the services provide integrated health and social care with one single point of access. For those who provide specialist health services only, referrals for adult social care learning disability services can be made via the generic social services referral processes.
