Achievements

Diabetes

  • NHS Diabetes Prevention Programme (NHS DPP) – Our network led implementation as a first wave NHS DPP site commencing in April 2016 for Nottinghamshire, Leicestershire and Northamptonshire.
  • Supported all East Midlands STPs in developing proposals for NHS DPP move to STP delivery from 2018/19.
  • Supported STPs in the development of applications for the Diabetes treatment and care funding. East Midlands STPs were awarded over £3.5m in transformation monies.
  • Support the Central Midlands and North Midlands DCO teams to undertake the assurance process for NHS DPP and Diabetes treatment and care.
  • Worked with NHS England central and regional communications in the development of a NHS DPP video to promote the programme across GP surgeries across England.
  • Following the successful pilot of Diabetes Prevention Week in the East Midlands, contributed to the national planning of Diabetes Prevention Week in April 2018.
  • Produced infographics for clinical commissioning groups to highlight trends and variation in diabetic foot amputation rates.
  • Completed an independent clinical peer review of the diabetic foot care pathway in Mid-Nottinghamshire, South Nottinghamshire and Northamptonshire. This has provided an in-depth review of the current pathway and recommendations for action to address identified gaps.
  • Commissioned a train the programme for diabetes foot risk assessments.

 Stroke prevention in atrial fibrillation

  • Updated atrial fibrillation infographics to highlight variation in AF diagnosis and management for clinical commissioning groups and STPs and to make the compelling case for action to improve diagnosis and management of atrial fibrillation
  • Providing tailored support to the six clinical commissioning group with the lowest anticoagulation rates to improve AF management
  • Working as a partner with the East Midlands Academic Health Science Network, Public Health England and other agencies to implement AF Advance Programme including roll-out of AliveCor mobile ECG devices in East Midlands to improve AF diagnosis and allow for early management in patients.

Cardiac services

  • Engagement with all cardiology heads of service to inform a cardiology services stocktake
  • Setting up Cardiac Services Clinical Advisory Group to support delivery of improvements in cardiac care and sharing of best practice
  • Survey of Heart Failure Pathways and Workshop in May 2018 to share models and facilitate dialogue between clinicians and commissioners on improving heart failure services and pathway models across the East Midlands.

Stroke

  • Stroke Clinical Advisory Group supports the development and improvement of stroke services across the region
  • A task and finish group has developed options for delivery of Mechanical Thrombectomy for consideration by commissioners.

Vascular

  • Vascular Clinical Advisory Group supports sharing of best practice and co-ordinates work to improve Vascular services across the region
  • Working with University Hospitals of Leicester to develop a business case to commission a structured education programme for patients with claudication. A pilot is proposed in Leicestershire with a view to support wider implementation across the East Midlands.
