Multispecialty community provider vanguards

There are 14 multispecialty community provider vanguards, working to develop a population based health and social care model.

These vanguards were selected following a rigorous process, involving workshops and the engagement of key partners and patient representative groups.

Multispecialty community provider framework

In 2015/16, during the first year of the new care models programme, the 14 MCP vanguards focused on defining and establishing their local MCP plans. In year two they focused on implementing, testing and refining these plans whilst measuring the impact on patients, staff and partner organisations.

Now in the third year of the programme, the vanguards are being supported to drive forward the delivery of their MCP’s. This includes fully implementing the care model as defined in the MCP framework and shaping approaches to commissioning for an MCP through contracting.

Many of the MCP vanguards have been involved in designing participation options for GPs and identifying the implications of these new organisational forms.

A further aspect for the vanguards during the final year of the programme is supporting others to learn from their experience, to expand and spread their care models through Sustainability and Transformation Partnerships (STPs), which are key to the future delivery of health and care services.
