End of Life Care

It is recognised that the early introduction of palliative care and advance care planning in a patient’s care can improve quality of life and reduce the use of inappropriate interventions or treatment at the end of life. People receive better, coordinated end of life care if they are identified early as being in the last year of life (Talking About dying: How to begin honest conversations about what lies ahead. October 2018 Royal College of Physicians).

This workstream covers all life-limiting diseases and conditions, with the regional team connecting with Mental Health, Cancer, and Urgent and Emergency Care Integrated Care Systems (ICS).  The regional work is supported by NHS England’s National End of Life Care programme and hosted by the Northern Cancer Alliance.
