Welcome to the web page for Advance Care Planning (ACP) for the Cheshire and Merseyside Palliative and End of Life Care Network.

This online facility has been designed to provide you with access to the ACP Framework and documents and some useful links to other resources.

If you are a member of the public you are welcome to explore the pages but be aware it is designed for professionals to use. You may wish to discuss the contents with one of your care team (e.g. General Practitioner or District Nurse).

What is Advance Care Planning?

Advance Care Planning is a process that gives people the chance to think about and write down what is important to them.

A person must have mental capacity* to make an advance care plan, and might find it helpful to talk to professionals about their care option (* for more information please visit Gov.uk and the National Archives)

As part of the process a person might choose to describe the type of care they would like at the end of their life.  For more information you may wish to look at the ACP Framework.
