Transforming participation in Chronic Kidney Disease

‘Think Kidneys’ – the NHS’s Transforming Participation in Chronic Kidney Disease programme has been developed to support and empower people living with chronic kidney disease and their families to achieve the personal and clinical goals that are important to them, wherever they are in the pathway of care. Patients, carers and clinicians will be valued and supported to develop the skills, knowledge and confidence to work together towards the best possible clinical and person-centred outcomes.

The programme is being coproduced with patients and carers who are working closely with their renal teams. We believe that some of the good practice developed through this programme will be transferable to other long-term conditions, meaning that more people can benefit from improved health and wellbeing.
