Torbay and Southern Devon NHS Foundation Trust Torbay enabled provision of a community and community hospital based stroke rehabilitation therapy service seven days a week. There was no additional cost involved in the redesigning of the service. Assessment of new patients is provided six days a week. Ongoing rehabilitation is provided seven days a week.
Lancashire Care NHS Foundation Trust has expanded its Chronic Obstructive Pulmonary Disease (COPD) service from five to seven days a week for community based patients and hospital in-patients. COPD emergency admissions have reduced by 12% and length of stay has reduced by an average of two days per admission.
Salford Independent Living Service, Salford City Council piloted a seven day equipment provision service saving 258 bed days in the six month pilot period. Patients and their families and carers have experienced safer, timelier discharge, and delayed discharges from the trust have reduced.
Western Sussex Hospitals NHS Foundation Trust provides a single point of integrated access for any clinician across the health and social care system. There has been a 3.7% reduction in the number of attendances to accident & emergency. There was a 2% increase in hospital stays for patients aged 65 and over and a reduction of 2.5% in the average length of stay for these patients.
