We are inviting frontline teams to collaborate with us to deliver and evidence the impact of personalised care in their service or pathway area.
By joining our ‘improvement collaborative’, teams will develop their knowledge and skills about personalised care, so they feel able to embed their own personalised care project into their clinical pathways. The collaborative will run until August 2020 and include 4 events to develop your skills in personalised care, to collaborate with your colleagues across the country on your own work, and spread your learning throughout their trust, and report back with evidence that will inform NHS England and Improvement policy going forward.
We are inviting organisations to nominate teams to be involved. We particularly want to work with multi-disciplinary teams that include AHPs and nurses and whose function it is to deliver care for people with long-term health and care needs, but other teams are very welcome to join too.
