The Model Acute Pathway provides evidence‑based standards to improve care during the first 72 hours of hospital care. Developed in partnership with the Royal College of Physicians, the Society for Acute Medicine and the British Geriatrics Society, the standards set out the practical actions needed to overcome persistent barriers and deliver meaningful improvement.
