NHS RightCare has recently published two new scenarios in its Long Term Conditions series.
Both scenarios – osteoporosis and wound care – use fictional people to examine patient journeys, comparing sub-optimal but typical scenarios against ideal pathways.
At each stage of the journey we have modelled the costs of care to commissioners and also described the impact of sub-optimal care and ideal care on the outcomes and experiences of patients.
The scenarios have been produced in partnership with clinicians and third sector organisations and show how the NHS RightCare methodology can help clinicians and commissioners improve the value and outcomes of care pathways. Both are available as detailed documents or summary slide sets.
