Preventing CVD by managing the high risk conditions

  • Hypertension, atrial fibrillation, high cholesterol, diabetes, non-diabetic hyperglycaemia and chronic kidney disease are high risk cardiovascular conditions.
  • Late diagnosis and under-treatment of these high risk conditions is common, and this substantially increases the incidence of stroke and heart attack.
  • Most of the diagnosis and management of these conditions takes place in primary care.
  • Systematic quality improvement across the high risk conditions is likely to have a cumulative impact in reducing incidence of stroke and heart attack.
