Primary Care Transformation
Primary Care services provide the first point of contact in the healthcare system, acting as the ‘front door’ of the NHS. Even with the significant achievements in this space to date Primary Care will need to build, embed, and further push transformation to meet the ever-changing needs of the population.
Primary Care has been on the front line of the COVID-19 response, with unprecedented effort making radical changes to how primary care services are delivered that would previously have taken years to implement, taking only a matter of months.
As the region moves out of the initial response to Covid, there is a great opportunity to springboard off of the huge improvements made and embed long term change in Primary Care to give the population of the South East the highest quality of care. The programme looks to support local transformation plans and other responses that the SE Region is making to the challenges currently faced in general practice. These will include enablers such as:
- Work with South East region ICBs to support local plans to deliver required changes
- Work with ICBs to co-commission for better value wherever possible
- Workforce programme support with new models of care
- NHS Digital programme support
