Medicine Use Reviews (MURs)

In most instances pharmacy contractors will provide MURs at their premises. There may however be exceptional circumstances where this is not possible, e.g. patient is housebound, in which case the contractor may apply for permission to conduct an MUR at alternative premises, at the patient’s home or by telephone. Please find attached the various forms that need completing and returning for prior approval in such circumstances:
