Trusted assessor scheme

The trusted assessor scheme can reduce delays to transfers of care.

Many people wait too long for discharge from hospital resulting in poor experience of the health and care system and poorer outcomes.

The guide ‘Developing trusted assessment schemes: essential elements‘ describes how hospitals, primary and community care and local councils could work together to deliver trusted assessment as a key part of the High Impact Change Model, as set out in the NHS Five Year Forward View Next Steps┬ádocument.
