An independent investigation into the care and treatment of a mental health service user Mr T in Cornwall
NHS England and NHS Improvement has published a mental health homicide review into the care and treatment of Mr T, who was a service-user under the care of Cornwall Partnership NHS Foundation Trust. Mr T took his own life in 2016 after being arrested on suspicion of murdering his baby son, who was subsequently found to have been unlawfully killed.
Cornwall Partnership NHS Foundation Trust has published an action plan on its website, in response to the report’s recommendations.
Kernow Clinical Commissioning Group has also published an action plan on its website.
Following the report on Mr T, an independent quality-assurance review on implementation of its recommendations has been published.

NHS England has published a mental health homicide review into the care and treatment of Mr M, who was a service-user under the care of Cornwall Partnership NHS Foundation Trust. Both Mr M and his wife were found dead at home in March 2016.
Cornwall Partnership NHS Foundation Trust, NHS Kernow Clinical Commissioning Group and Outlook South West have published the associated action plan on their websites.
Following the report on Mr M, an independent quality-assurance review on implementation of its recommendations has been published.
