The government has provided a national discharge fund via the NHS, for quarters 1 and 2 of 2021/22, to help cover some of the cost of post-discharge recovery and support services/ rehabilitation and reablement care following discharge from hospital. This guidance applies to the period of 1 April 2021 to 30 September 2021.
