The Coalition for Collaborative Care (C4CC) was set up to help make person-centred care for people with long-term conditions a reality. The C4CC brings together like-minded organisations, including NHS England, who want to bring about change, they work in co-production with people who have lived experience of long-term conditions.
This innovative partnership of more than 45 national organisations across the health, social care, voluntary and community sectors is focusing on changing the relationship between people with long-term health conditions and the professionals supporting them. This allows the expertise of both to be used most effectively to help people manage their condition and maximise their wellbeing.
