The request was for a detailed summary of incidents taken from reports of Never Events for the 2016-17 financial year on the NHS England website and a list of provisional Never Events for April 2017 to 31 August 2017.

NHS Improvement held the information requested and released it subject to withholding information on the basis of the exemption at section 40 of the FOI Act.
