The request was for further details in relation to information previously released about delayed breast cancer diagnoses including why some incident reports stated that patient sex was not stated/unknown; why some incident reports had missing information on patient gender and whether medical professionals can alter an incident report at a later date to take account of any changes in the patient’s prognosis.
