The request asked for the number of documentation incidents which were recorded in the 2016/17 where the degree of harm was recorded as “death”, with a summary of each documentation incident.

NHS Improvement held the information requested and released most of it. Some of the information was withheld on the basis of sections 31, 40 and 41.
