Summary

Findings from this nurse-led study indicated substantial variability in specialist nursing roles and the extent of role development, which impacted upon how services supported vulnerable patients with complex psycho-social and/or medical needs, to keep them engaged in care and prevent hospital admission. This study highlights evidence of unwarranted variation in service provision and the need for services to make better use of resources for better outcomes for individuals.
