Taurus Healthcare Ltd federation brought together the Time for Care and Primary Care Home programmes to improve collaborative working between general practice and health and care teams across its locality, helping for the first time to redesign primary care in its widest sense.

Summary

Through the work social prescribers were employed that are shared between localities, and who during the first six months received 524 referrals from across the practices, which previously would have resulted in inappropriate GP appointments.  Time for Care is part of the support available through the General Practice Forward View.
