This diagram shows a number of elements of an optimal CVD prevention pathway including the evidence-base, clinical interventions, information on the risk conditions and potential opportunities for improvement.
By clicking on the boxes in the pathway users can link through to further information such as case studies, NICE guidance or detailed high value interventions. Local health economies may find this optimal value pathway useful during the What to Change and How to Change phases of their NHS RightCare work.
For further information about CVD indicators, variation and the size of opportunities (both in terms of clinical and financial outcomes) local health economies are encouraged to use this pathway in conjunction with the Commissioning for Value focus packs on CVD, which have been produced for every CCG area.
