If you wish to apply for a Mandatory contract, additional contract or both contracts please give NHS England at least 12 weeks’ notice to process your application .  Please see guidance to determine which application form and sections need to be submitted along with copies of DBS certificate, two professional references, and CV for each Director, Company Secretary and each performer to NHS England South (South West) Eye Care team:

The point of contact for all performers’ list changes and updates is via Primary Care Support England (PCSE).

Please remember to submit GOS4 forms for over 16s with a particular medical condition initially to NHS England South (South West) Eye Care team, Peninsula House, Kingsmill Road, Tamar View Industrial Estate, Saltash PL12 6LE for an optometric advisor to authorise.

Please ensure that your supplier name, address and telephone number are listed on the GOS4.  In addition, it is imperative, that the correct benefits that patients receive is indicated on the form.  The authorised / non-authorised form will then be returned to your practice.  If authorised you will need to submit to PCSE for payment.

Quality in Optometry (QiO)

Further Quality in Optometry (QiO) verification visits will be arranged and practices notified accordingly.  These should be carried out by Christmas 2018.
