This document sets out to raise awareness of the supportive and palliative care needs of people living or dying with progressive heart failure, to facilitate the commissioning of services specifically tailored to meet those needs.


This report was published by NHS Improving Quality, incorporating the knowledge and expertise from the NHS National End of Life Care Programme. The National End of Life Care programme produced the route to success series to provide staff with simple guides to support the implementation of the End of Life Care Pathway. The framework promoted supportive and palliative care for people with heart failure.
