These workbooks are for panel members to record their findings during a Care and Treatment Review (CTR) or Care, Education and Treatment Review (CETR), based on the key lines of enquiry in the Key line of enquiry templates (KLOE) template. The workbooks are easy to follow and support panel members to be involved in gathering the information needed. The CTR/CETR chair should bring copies of the relevant workbooks to a CTR/CETR and collect them at the end of the CTR/CETR to ensure safe storage and disposal.
