The purpose of this data collection sheet is to enable the consistent exchange of patient level information between provider and commissioner in support of the commissioning of PbR excluded devices. Whilst this report format has been defined specifically to support the commissioning of Prescribed Specialised Services, this report  format could potentially be used by other commissioning functions.
