This page includes consolidated guidance from 2019 to support NHS foundation trusts and NHS trusts to understand the previous provider licence requirements under G6, FT4 and CoS7.  The NHS provider licence was updated in April 2023 and trusts and foundation trusts are no longer required to self-certify under G6 and FT4, however those providers who have a service designated as a Commissioner Requested Service should continue to self-certify against the CoS7 requirements and should send this reporting to their region.
