NHS Excellence Awards – Meet Our Regional Winners

Teams and individuals across North East and Yorkshire have been named as regional champions in the first NHS Excellence Awards, recognising work that is improving care for patients and communities. The NHS Excellence Awards are the only awards run by, and for, the NHS. They recognise, celebrate and share innovative work taking place across health and care.

The awards shine a light on local teams, projects and individuals who are driving improvements, with the aim of sharing learning and inspiring others to adopt successful approaches in their own areas.

These regional champions have been recognised by regional panels for their innovative and collaborative work in their local areas, highlighting digital projects, quality improvement initiatives and examples of excellent leadership.

▼ Delivering Value Award – Transforming Same Day Urgent Care and Winter Services across North Yorkshire – Nimbuscare

Nimbuscare is being recognised for transforming same-day urgent care across North Yorkshire. Designed for direct booking via general practice and delivered in neighbourhoods, this laid the foundation for North Yorkshire’s first fully integrated 24/7 urgent care service. By shifting from walk-in to GP-bookable appointments, the service has streamlined the patient journey, offered alternatives to A&E, and generated significant cost savings. With high patient satisfaction, the model integrates Pharmacy First and a local Clinical Assessment Service bookable via NHS111, offering a scalable, efficient, and value-driven approach to urgent care that aligns with national policy.

Dr Tori Blake, Director of Urgent Care, said:

“We’re delighted to receive this award, which recognises a fundamental shift in how urgent care can be delivered. By integrating general practice, Pharmacy First and NHS111 into a single neighbourhood model, we’ve significantly improved access, reduced unnecessary hospital attendance, and ensured patients are seen quicker and closer to home. At its core, this is about delivering value across organisational boundaries, using our collective expertise and resources for the benefit of patients.”

 

▼ Quality Improvement Award – Gold Team Approach to Improving Palliative and End of Life Care in General Practice – Holderness Health

The Gold Team Approach at Holderness Health is being recognised for transforming palliative and end-of-life care in general practice. By addressing gaps in continuity and communication, they developed a comprehensive model including a rapid-access Gold Line, proactive GP contact, and a support pack for families. Their work has reduced call waits to under a minute, improved patient identification, and earned praise from families and partners alike. The approach has enhanced staff morale and created a connected, compassionate system, with the model already being shared for wider adoption.

Valuing our People Award – North East and North Cumbria Staff Mental Health and Wellbeing Hub – North East and North Cumbria ICB & Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust. 

The North East and North Cumbria Staff Mental Health and Wellbeing Hub is being recognised for its outstanding commitment to valuing its workforce. Supporting 175,000 health and social care staff with rapid, barrier-free access to mental health support, the Hub has improved wellbeing, reduced anxiety and depression, and helped staff remain or return to work. With self-referral, multiple therapy options, and compassionate, rapid responses, it has become a model of best practice. The Hub’s success is driving a long-term, integrated approach, ensuring sustained impact for staff wellbeing and patient safety, with potential replication across other sectors.

Working in Partnership – Marie Curie Responsive Emergency Assessment and Community Team (REACT) – Marie Curie Hospice, Bradford 

‘The Marie Curie REACT team is recognised for their pioneering partnership in delivering acute, high-quality palliative care in Bradford. By identifying patients in crisis at the Emergency Department and providing rapid community support, they have reduced hospital bed days from 38 to 17, saved over 20,000 bed days, and improved equitable access to care. Their collaborative, holistic model has changed attitudes towards palliative care, and the approach is now fully funded and being replicated, ensuring more patients receive compassionate end-of-life care in their community.’

Improving Health Outcomes – Wakefield Family Nurse Partnership Team – Harrogate and District NHS Foundation Trust.  

The Wakefield Family Nurse Partnership Team is being celebrated for their innovative and data-driven approach to improving health outcomes for young parents. Delivering the Family Nurse Partnership programme, they focus on early intervention, enrolling vulnerable parents early in pregnancy. Their efforts have led to smoke-free homes, full immunisation rates, and reduced unplanned pregnancies, all surpassing national averages. By creating tailored pathways for contraception and smoking cessation, they’ve fostered sustainable change. Their commitment to partnership, trauma-informed care, and training the wider workforce has amplified their impact, giving young parents and children a stronger voice and reducing inequalities.

 

Dr Hamish McLure, Regional Medical Director for NHS North East and Yorkshire, said: “Picking our finalists from the hundreds of high-quality nominations we received from all across North East and Yorkshire was incredibly difficult, so all of those who are going forward to the national ceremony should be extremely proud of their achievement. Regional champions from across England will now form the shortlist from which national winners will be selected. Winners will be announced at an awards ceremony at NHS ConfedExpo in Manchester in June.
