Our national Stop the Pressure programme has developed a guide to support nurses and other healthcare professionals in preventing pressure ulcers. The recommendations in our guide will support an organisation’s ability to learn from reported incidents and looks at ways to improve the prevention of pressure damage.

The recommendations in our guide will support you to deliver a consistent approach to defining and measuring pressure ulcers as well as helping you to understand the level of pressure damage harm in England. The recommendations are for implementation from April 2019.

Guidance for reporting pressure ulcers
This guide is intended to help tissue viability nurses/pressure ulcer prevention leads to work with their risk/governance teams. It aims to help these teams ensure the changes required by NHS Improvement’s Pressure ulcers: revised definition and measurement framework (November 2018 ) are embedded in their local incident reporting system fields whilst maintaining correct connectivity to the National Reporting and Learning System.
