Neighbourhood health – case studies of good practice

These case studies provide examples of existing good practice that forms the foundations of neighbourhood health. They should be read alongside the Neighbourhood health guidelines 2025/56.

Currently, there are no known examples of systems delivering all the initial 6 core components of neighbourhood health (detailed in appendix 1 of the Neighbourhood health guidelines) in a coordinated, consistent way or at sufficient scale to support the necessary improvements in health and system efficiency.

For 2025/26, we ask all systems to consider how they can increase the consistency, integration and scale in delivering health and care to adults, children and young people with complex health and social care needs who require support from multiple services and organisations.

As in the example of 16-year-old Justina, this should include implementing any necessary reasonable adjustments (as required by the Equality Act 2010 and supported by the reasonable adjustment digital flag). This will accommodate the needs of people with disabilities, improving their health outcomes and experience of health and care services, and reducing their risk of premature mortality.
