Medicine Use Reviews (MURs)

In most instances pharmacy contractors will provide MURs at their premises. There may however be exceptional circumstances where this is not possible, e.g. patient is housebound, in which case the contractor may apply for permission to conduct an MUR at alternative premises, at the patient’s home or by telephone. The various forms that need completing and returning for prior approval in such circumstances are available on the Pharmaceutical Services Negotiating Committee (PSNC) website.

Application for doctor dispensing of medicines on the grounds of serious difficulty

Use this form for patients who live within 1.6km (1 mile) of a pharmacy who are not normally eligible, but wish to apply to receive their medicines from their doctor’s surgery (note that the distance is taken as a direct radius / line rather than mileage by road).

Patients are to fill in the form and send to their GP for completion.  Completed forms should be sent to the address provided.  On receipt, they will be forwarded to local Pharmaceutical Services Regulatory Committee for consideration. All applications will be considered on an individual basis and only approved in exceptional circumstances.
