The Community Healthcare Operational Data Flows (Community HODF) is a pilot testing daily collection of a core subset of the Community Services Data Set (CSDS). The aim is to improve the timeliness of data available for care planning, operational management and patient outcomes.

Only specified providers of publicly funded community health services are currently submitting data as early adopters. If your organisation would like to take part, contact england.fdf@nhs.net.
