Ageing Well covers the Frailty, Dementia and the Palliative and End of Life Care Clinical Networks.

Each Clinical Network has its own programme of work, and the Ageing Well Networks work together on shared priorities such as the Universal Care Plan. Each Network has its own Clinical Leadership Group, which is a multi-disciplinary forum for clinicians and key stakeholders to meet and share their expertise and experience in order to improve the quality, and inform commissioning, of health services across London.
