Ambitions for end of life care

Dr Paula Powell, Clinical Lead, Cheshire and Merseyside Palliative and End of Life Care Clinical Network said: “The Cheshire and Merseyside Palliative and End of Life Care Clinical Network works with clinical and commissioning  colleagues to reduce variation and promote equity and excellence in end of life care to support people to live well before dying with peace and dignity in the place of their choice.

The network will work with its partners across Cheshire and Merseyside in responding to the Ambitions for End of Life Care and making those ambitions a reality for the people and those who care for them.”

Self-Assessment Ambitions Framework

To this end, the network commissioned a self-assessment framework. This has been presented to each locality at their Integrated Clinical Network (ICN) meetings, where members from the Hospice, Hospital and Community Specialist Palliative Care teams, along with the Commissioners, discuss local issues.

The framework will enable them to score their end of life care services against each ambition.  It is hoped that this will provide an opportunity for each locality to meet, self-assess and provide a benchmark to improve end of life care.

In addition there is also a dedicated website to which we can all point and which emphasises the shared system-wide ownership.
