Patient safety review and response reports

Six monthly summaries of how we reviewed and responded to the patient safety issues you reported.

Our new ‘Using patient safety events data to keep patients safe’ webpages

We no longer publish the Patient safety review and response reports. This information is now available on our new ‘Using patient safety events data to keep patients safe’ webpages, which also feature the most up-to-date case studies.

The previous review and response reports are still available via the links below.

What do the patient safety review and response reports cover?

Each of our Patient safety review and response reports covers a six month period, outlining how we identified issues and risks by reviewing patient safety incident reports, and the actions taken as a direct result to protect patients from harm. They include information and case studies on what we did to address ‘rare and under-recognised’ safety issues identified through the National Reporting and Learning System (NRLS) and other sources.

First and foremost, by showing the difference their efforts make, these reports are a thank you to all the staff, patients and members of the public that take the time to report, but also provides useful information about the core work of our patient safety team and how it uses incident reporting to improve safety across the NHS.
