This template has been introduced to aid when NHS England, Education Provider and Placement Provider wishes to entire into Tri-partite agreement in connection with the service set out in this agreement.

This template is generic form which can be used by holders of the NHS Education Funding Agreement to manage placement agreement where an agreement does not exist between parties.

This template letter should be used to facilitate the coordination of Schedule 4A – Undergraduate Dental Education Tri-partite Agreement and Schedule 4B – Undergraduate Medical Education Tri-partite Agreement with medical or dental schools, placement organisations and NHS England.
