Urinary catheter tools
The urinary catheter tools have been developed collaboratively with national experts to support providers in delivering consistent evidence based catheter care. The use of the tools needs to be supported by strong leadership and education.
Catheter passport
This may be printed as one document or in separate patient and clinical sections. We recommend you use one passport consistently across organisational boundaries. Regional leaders are encouraged to engage ambulance providers to ensure that passports are effectively transported with patients.

Catheter card
The catheter card offers an alternative option when patients will not use a catheter passport.
- Catheter card – Developed by the Shropshire and Telford and Wrekin Local Health Economy E.coli blood stream infection reduction group, as a collaborative approach from the following organisations Shropshire Community Health NHS Trust, Shropshire CCG, Telford and Wrekin CCG, Robert Jones and Agnes Hunt Foundation Trust, Shrewsbury and Telford Hospital NHS Trust, Shropshire Partners in Care.
Inpatient care plan
This document has been designed for use in all inpatient settings (including long-term care and community hospitals), although parts can be adapted for use in a patient’s home. The trial without catheter can be conducted in a clinic or ward setting.
No catheter-associated urinary tract infections (CAUTI) card and poster
These have been designed to support an educational programme to reduce catheter-associated infection. They are for staff use only and assume concurrent teaching or prior knowledge.
