If you’ve been diagnosed with high blood pressure, you may be eligible to take part in the NHS Healthier You programme, designed to help you make lasting lifestyle changes to help reduce your risk of serious health problems.
If you have high blood pressure, you can join a free NHS programme that helps you make healthy changes to protect your future health.
Why have I been invited?
Having high blood pressure, also called hypertension, increases your chances of developing long-term conditions like:
- heart disease
- stroke
- chronic kidney disease
- dementia
The good news? Making small, positive changes to your lifestyle now can help you stay healthy for years to come. Remember that without support you might be at risk of developing these long-term conditions.
That’s exactly what the Healthier You programme is here to support you with.
How does the Healthier You: Long Term Conditions Prevention Programme work?
Healthier You is a 9-month behaviour change programme funded by the NHS. It offers friendly, personalised support to help you:
- eat more healthily
- move more
- reach or maintain a healthy weight
You can choose how you take part:
- face-to-face group sessions in your local area, led by trained coaches
- online through an app, so you can do the sessions whenever suits you best
What happens once you sign up?
- your Healthier You provider will contact you to arrange a time to talk about the programme
- during this call, you’ll hear more about the different ways you can take part, either in-person at a local venue or digitally through an app
- you’ll also have a short individual assessment to help tailor the programme to your needs
- if you choose face-to-face sessions, you’ll be given options of date, time and location of your group sessions
- if you choose the digital option, you’ll get login details and access to one-to-one support through the app
- there’s no need to prepare anything in advance
Please note that the programme is not intended as a treatment for hypertension. If your blood pressure is not controlled, we recommend that you speak to your GP about managing your blood pressure.
How do I get started?
It’s quick and easy to join. If you have a high blood pressure diagnosis and received a message from your GP practice, follow these steps:
- select the area you live in from the list below
- click on your provider to sign up
- remember, you don’t need a GP appointment to join
