Prevention – the National Diabetes Prevention Programme (NDPP)

The NWC  Clinical Network Diabetes team are integral in the implementation and running of the NDPP. We have coordinated the NWC implementation of ‘wave 2’ to ‘wave 5’ NDPP, ensuring that all CCGs and STPs in the NWC region are engaged in the programme.

There is strong international evidence to demonstrate how behavioural interventions supporting people to maintain a healthy weight and become more active, can significantly reduce the risk of developing the condition. The NDPP aims to reduce individuals’ risk of a range of conditions linked to sedentary lifestyle, poor nutrition and being overweight or obese by working with people who have been identified as having an appropriately elevated risk level of blood glucose level (HbA1c or FPG).

There is full coverage of the programme across Cheshire and Merseyside. The network facilitates meetings and workshops with all stakeholders including the provider, whilst underpinning the programme’s governance structure.

In Lancashire & South Cumbria the lead CCG works closely with the North West Coast Clinical Network and Public Health, Commissioning Support Unit and the Provider to enable the successful referral of our patients onto the programme.
