Community rehabilitation toolkit
Community rehabilitation services are uncoordinated and inconsistent, supporting data to help improve services is lacking and the workforce is often insufficient to meet current need.
Health systems that experience these issues can use our new toolkit as a key resource in helping to improve community rehabilitation services.
The Toolkit will support systems to understand the priorities for community rehabilitation services and the key actions to take. It provides opportunity to assess and benchmark current systems to find opportunities for improvement.
Commissioners responsible for community rehabilitation services for their population should focus on the following system improvement opportunities:
- Population identification and segmentation based on symptoms, function and need
- Supporting people to stay well and maintain independence
- Prevention of escalation and restoration of previous function
- Supporting the hospital / community interface
- Integrated approach to commissioning across the whole system
- Match workforce to population needs
- Improving data for rehabilitation services
- Timely access to technology, facilities and patient equipment
- Person centred care and experience of care
