This is the report of the independent investigation into the care and treatment of a mental health service user Mr J in Kent.

At the time of the homicide (2018), Mr J was under the care of the Early Intervention in Psychosis team at Kent and Medway NHS and Social Care Partnership Trust.

An independent review into the care and treatment of Mr D: April 2019

This is the report of the independent review into the care and treatment of Mr D who was convicted of murder in November 2017.

At the time of the homicide (May 2017), Mr D was not receiving mental health services. He had received mental health support from Kent and Medway NHS and Social Care Partnership Trust from May 2015 until February 2017.

Kent and Medway NHS and Social Care Partnership Trust has published the associated action plan.

An independent inquiry into the care and treatment of Mr X: July 2016

This is the report of the independent investigation into the care and treatment of Mr X who was convicted of manslaughter on the grounds of diminished responsibility.

At the time of the homicide (2011), Mr X was receiving mental health services provided by Kent and Medway NHS and Social Care Partnership Trust.

Kent and Medway NHS and Social Care Partnership Trust has published the associated action plan.

An independent inquiry into the care and treatment of Mr D: November 2014

This is the report of the independent investigation into the care and treatment of Mr D and a follow up review.

At the time of the homicide (2011), Mr D was receiving mental health services provided by Kent and Medway NHS and Social Care Partnership Trust.

This is the report of the independent investigation into the care and treatment of Mr P. At the time of the homicide (2009) Mr P was receiving mental health services from Kent and Medway NHS Social Care and Partnership Trust.
