NPL 3:  National performers lists change notification form: Change in registered address, practice within existing locality or status of inclusion

GPs should initially complete and submit an NPL3 where they are changing name; change of address (home); change of status (e.g. partner to salaried GP); transfer of practice within same NHS England Area Team locality or resignation or retirement.
