South East Stroke Standards
The South East Stroke Clinical Advisory Group has reviewed the South East Stroke Standards following the recent publication of the revised RCP Stroke Guideline (2016) and the amended version is now available.
South East Stroke Service Specification
The South East Stroke Service Specification has been recently updated following the publication of the revised RCP Stroke Guidelines in September 2016. The specification looks at the whole stroke pathway with the purpose of supporting the delivery of high quality care to patients and to achieve the standards set out in the South East Stroke Standards
The Sentinel Stroke National Audit programme (SSNAP) TEST
SSNAP is a longitudinal prospective audit which collects a minimum dataset for every stroke patient to measure processes, outcomes (at 6 months) and care in the community.
The 5 stroke indicators from the ICB Outcome Indicators Set are all included within SSNAP, which also provides data for the NICE Quality Standard, the NHS Outcomes Framework, and metrics aligned with those in the Cardiovascular Disease Outcomes Strategy. Life After Stroke Guidance
The South East Cardiovascular Clinical Network has the pleasure of launching the Life after Stroke Commissioning Guidance for ICBs and Local Authority Commissioners.
This guidance has been developed through the CVD life after Stroke task and finish group which comprised of patients, carers, Integrated Care Board and Local Authority members and voluntary organisations, as well as a wide range of healthcare professions from acute and community trusts, private, and social enterprise providers – all of whom were directly involved in developing and shaping this work.
We hope this information pack will be of use to you, and will ultimately help in the commissioning of Life after Stroke services for patients across SE.
