- High impact actions for service improvement and delivery by Transforming Care Partnerships (published November 2016) – this document is intended to be complementary to the work on new models of care that is already underway and supports Transforming Care Partnerships (TCPs) to go further and faster by providing a focus for every member of the team to play their part to ensure their local care system is working effectively.
- Building the Right Support: Finance frequently asked questions (updated September 2016) – this provides frequently asked questions on financial arrangements to support commissioners and Transforming Care Partnerships in their planning process, these replace the original FAQs.

- Care and Treatment Reviews – toolkit for commissioners (published October 2015) – this toolkit has a range of materials to support commissioners on Care and Treatment Reviews, and includes information in easy read for people with learning disabilities and/or autism and their families.
- Transforming Care for People with Learning Disabilities – Next Steps; progress report (published July 2015) – this document updates on the work that is being delivered by the national Transforming Care Delivery Board partners: NHS England, Local Government Association, the Department of Health, Association of Directors of Adult Social Services, Care Quality Commission and Health Education England.
- Transforming Care for People with Learning Disabilities – next steps (January 2015) – this is the joint action plan agreed by NHS England, Local Government Association, Department of Health, Association of Directors of Adult Social Services, Care Quality Commission and Health Education England, to transform services for people with learning disabilities and/ or autism.
- Winterbourne View – Time for Change (published November 2014) – this report was commissioned by NHS England and outlines Sir Stephen Bubb’s recommendations to transform the commissioning of services for people with learning disabilities and/or autism.
Improving health
- 2015/16 General Medical Services (GMS) contract guidance for GMS contract 2015/16 – this is guidance for GP practices on implementing the General Medical Services contract for 2015/16, including the health checks for people with learning disabilities.
- Reducing premature mortality in people with Learning Disabilities: Effective interventions and reasonable adjustments – this is joint guidance from NHS England and Improving Health and Lives Learning Disabilities Observatory on making reasonable adjustments to acute hospital and primary care services, for people with learning disabilities.
- Letter about joint Health and Social Care Learning Disability Self-Assessment Framework 2015 (published November 2015) – this letter outlines changes to the Joint Health and Social Care Learning Disability Self-Assessment Framework for people with learning disabilities.
- Clinicians’ letter – This is a template letter which clinicians can use to inform GPs about patients with a learning disability, to ensure young people with SEND are included on the GP register of learning disability, where appropriate. This means they are automatically called for an annual health check from the age of 14, and that reasonable adjustments can be made when the person attends other health settings.
- Parents’ letter – This is a template letter which parents/carers can also use to ask their GP to ensure their child is added to the GP register.
